Click Here to log onto the membership site.

Step 2
Fill out the form on the Welcome page. Your
Username and Password must be at least 5 characters long.
Please note that the only information that is
required is your name and email address. I would encourage you to
give as much information about yourself as you are comfortable
with. If you would prefer to get to know and trust me
first, you can always add to, update or change your information at
a later date.

Step 3
Check the box indicating you agree to the
Terms of Service and fill in the Verification Code.

Click on the Join Now button.

Step 5
You will then be taken to a Congratulations! page.
However, you will have to verify your email address with AWeber before you can
download your Gift Certificate.
So, check your inbox. You will receive an email
that includes a confirmation link you must click to validate your
account. When you click this confirmation link, you will receive a "Welcome" email. This "Welcome" email will contain a link for accessing your Gift Certificate.

However, you must first log onto your membership:

After you log into the Toxic Free Membership site, use the link provided in your "Welcome" email. This will take you to the Certificate Code page on the website.
Enter the code
Gene53 in the code box (You can use upper or
lower case).

Step 7
Your browser will open a pdf file containing your Gift
Certificate. Print this file, fill it out completely and either
bring it to my dental office when you come in for a
visit, or use it towards a phone consultation with me
concerning your dental health issues. |
